230sl Manual Steering Gearbox

I have gone through my entire front steering system because I have been getting a knocking noise and some play when the steering wheel is turned full lock with the car stopped. It happens when beginning to turn the steering wheel the opposite direction and the steering is loaded.

I've looked and tested everything, including looking at the steering coupler bushings which are fine. 

The clunk is definitely coming from inside the manual steering box, but only when it has a load on the steering. It can't be recreated with the car in the air.

What exactly in the box is usually wrong with it? Is it safe to drive it (please no OMG overreactions), and what is the best place to get a rebuild? I have searched the forums like crazy.
