Original Key holder or not?

Does anyone know if this could be an original key case for an early 250 sl?

Could be.  Friend and I made 190 SL fob repros and they were similar for 230/early 250, I think.  I use one of my repros for my early 250

How long have they been making repros
I have had this for almost 20 yrs and it is very worn??

One of our members Michael S. made a reproduction of these about ten years ago.  they were exactly as per the photo. Still have one.

Sure looks very similar to the one MB sells now as p/n 1088900661 for $35 or less.

This is the key case that came with my 1966 230SL when delivered in Feb. 1966.  I do not know if it was Mercedes supplied or dealer supplied.  I think there was a Mercedes star on the opposite side but it is so worn, I cannot tell for sure.

 Never used this Key Holder that came with my 280SL. Don't recall if my '67 250SL had one or not.

49er's was used on the 190 SL, not sure if 230 and early 250.

I am pretty sure that the key holders were dealer-supplied items. These things are usually part of a promotional item assortment available to the dealers from the factory and are used on all vehicles, not one particular model. One of my BMW motorcycles has an almost identical little pouch, sans the Mercedes star, of course.

Here is the well used one that I am pretty sure came with my car at the factory (February, 1969 delivery).  My shipping records from shipping the car to the USA in 1970 indicate that the set of keys provided to the shipper were missing when the car was collected in New Orleans. The shipping insurance paid for a replacement set of keys.  I therefore only have one of these. 

Please note the snap fastener marked “Stocko 2”.  Stocko was a German manufacturer of snaps founded in 1901 and the company is still in business today as a part of YKK-Stocko, selling all sorts of similar fasteners.
